Add unique index to uuid column in concept_reference_map and concept_reference_source tables

Description

This seems to be a standard index added to uuid, but it's not present on these two tables.

In particular, adding this index adds a major performance improvement to importing metadata sharing packages.

does anybody see issues with adding this and backporting down to 1.10.x
