Background

Prior to conducing a usability test it is advisable to have a consent form signed.

For the purpose of OpenMRS 

Please have the user sign this form

Extra Background

The goal us to make the consent and permission to record form as brief as possible. 

Therefore our form excludes sections that the usability testing form for other organizations may include: such as the non-disclosure agreement.

Because the work of OpenMRS is open-source, our usability testing forms do not induce a Non-Disclosure Agreement (NDA). An NDA is more common to be included in usability testing that involves startups and organizations that have proprietary information they do not want shared ahead of time.

Please introduce the form using the usability testing Script.

It states, "We will be recording your session to allow those who are unable to be here today to benefit from your comments. Please read and sign if you agree. (hand form to user)"
