Care and Treatment Service Enrollment Encounter Type

An interaction in which a client is registered for care and treatment services.

ART Therapy Encounter Type

An encounter that focuses on the assessment and management of a client's antiretroviral therapy (ART) treatment. The encounter involves evaluating ART side effects and making any necessary adjustments.

Service Delivery Model Encounter Type

An encounter for assessing the patient's HIV needs and preferences to ensure that care is coordinated and aligned with the patient's overall treatment plan.

Transfer Out Encounter Type

An encounter that focuses on a patient transfer from one healthcare facility to another, to facilitate a smooth transition and continuity of care.

Patient Tracing Encounter Type

An encounter focused on locating and reaching out to patients who have missed appointments or are non-adherent to their treatment plans. This encounter involves documenting efforts to contact the patient and facilitating their return.

HIV Death Encounter Type

An encounter that documents the circumstances surrounding the death of a patient with HIV. This encounter includes recording the date and cause of death.

Clinical Visit Encounter Type

An encounter type that involves a patient’s appointment with a healthcare provider for assessment, diagnosis, and management of their HIV condition. The encounter includes documentation of patients' physical examinations, lab tests, and HIV treatment, all aimed at providing comprehensive care and monitoring the patient's HIV status.

Mental Health Assessment Encounter Type

An encounter focused on evaluating a patient's mental health status to diagnose mental health conditions, monitor severity, and inform appropriate treatment plans to support the patient’s overall health.

Disclosure Encounter Type

An encounter that involves counseling sessions focused on the disclosure of an HIV diagnosis to patients, particularly children and adolescents. This encounter includes age-appropriate information sharing to ensure that the disclosure process is handled sensitively and effectively.

Contact Tracing Encounter Type

An encounter aimed at identifying and reaching out to individuals who may have been exposed to HIV through an index client. This encounter involves documenting details about the contact, conducting risk assessments, providing information about testing and prevention services, and facilitating access to HIV care services.
