Form Bank
Requirements for the Metadata Sharing Server are being gathered. It is designed to replace this page with a better tailored approach. If you have any suggestions, please visit the project page and contact us.
This form bank is an exchange area between OpenMRS implementers to share electronic forms. Please consider exporting your forms with the Metadata Sharing Module and sharing them here. Contributors should attach the OpenMRS form to this page, along with an optional original paper form. It is recommended that metadata sharing module is used to create a zip file which contains the form along with all dependencies (concepts, datatypes, classes, etc). The form could be htmlform, Xforms or an Infopath form. To use a form from the form bank, you will need to download and install the Metadata Sharing Module. You can find instructions for importing metadata (the XML files on this page) in the User's Guide for Metadata Sharing.
Forms can now be exported between various versions of OpenMRS. See OpenMRS ticket [META-44] and [META-168]
