Cohort Module Enhancements

Enhance work done on Cohort data capturing uptill now.

Features should include

1. Cohort based dashboard where

   1. Data is diaplayed by Cohort (aggregate)

   2. By Individual Patients belonging to the cohort (granular)

2. Extensive searched mechanism available over an extensive range of parameters

3. Extend Integratation of Module with HTMLFormEntry with proper tags (half of the work is already done)

4. Redesign UI to make user experience better

5. Integration with Reporting Module
