•  The fulfiller status and fulfiller comment on Order could be used for this, with the fulfillerStatus set to EXCEPTION (does this make sense?) and the fulfillerComment could capture any free-text.  However, if we'd need to capture a coded reason we'd need to figure out where we'd want to store that
